Patient Information

INSURANCE AND PAYMENT INFORMATION

Gateway Center for Lymphedema Care is an out-of-network provider for insurance plans. We can only receive payment from insurance plans that cover providers who are out-of-network and from managed Medicare plans with pre-verification.

 

We will submit claims on the patient's behalf if the insurance is pre-verified. The patient will be responsible for any co-payments at the time of service. In most cases, insurance companies will pay us directly.

 

In certain instances, the insurance companies may reimburse the patient directly. We have no problem submitting the claims for the patient, but the patient will be responsible for the full fee at the time of service.

 

SELF-PAY PATIENTS

Payment is expected when services are rendered.  We do offer payment packages for multiple visits. For your convenience, we accept cash, check, MasterCard, Visa, and Discover.

 

 

CANCELED/MISSED APPOINTMENTS

We require 24-hour notice for cancellations.  Appointments that are canceled with less than 24 hours’ notice are subject to a $100 charge, which is not reimbursable by insurance companies. Late arrivals and no-shows are subject to the full fee for the session.
